Overview of Consent Forms

It is important to understand the principles of Consent or Standard Forms.

 

We supply with each of our main systems a set of Standard Forms , in this case Consent Forms. These are forms that a client can sign for a treatment to provide permission for treatment or Euthanasia (if applicable).

 

These forms can be changed to suit your own practices and subsequently saved BUT they are not intended for everyday use. As such they are the base, skeleton file or master file. To create a file for every day use you need to save the master as a different file.

 

By way of an example, every system comes with a standard form called General Consent. Once you have modified that file to your own tastes you will then save that file. You can call the file you are creating for everyday use General Consent or perhaps Consent for ABC procedure. You can then save the master again as perhaps Consent for XYZ procedure. Both versions might be slightly or very different. This way you can build up a comprehensive set of Consent Forms and even create completely new forms for your own use
